CCA utilizes a digital platform for eligibility which is directly linked with the FHSAA. Please follow the steps below to create an account and upload paperwork.

FHSAA Form EL1 – ECG Screening
FHSAA Form EL2 – Pre-Participation Physical Examination
In addition to the forms above, the FHSAA requires all student-athletes to complete the NFHS Concussions Course, Heat Illness Course, Sportsmanship Course, and Sudden Cardiac Arrest Course prior to participating in tryouts or practice.
